The problem
New students hand over a paper health declaration at the door or, in a rush, say it out loud, and teachers read it, if at all, as class starts. If 25 new students a month each take 5 minutes to fill in, type up and file, that's 125 minutes, about 2.1 hours.
What the AI does
Build a short Google Form asking about injuries, medical conditions, pregnancy and medication, with a line that the teacher will read it and may talk to the student before class. Responses go to a Google Sheet that only teachers and the owner can see. A two-step Zapier automation starts on each new row and emails the student a receipt that the form was received; it does not include their answers. The teacher reads each response before the first class. The form records what the student says; it does not decide anything. The teacher decides what is suitable, or advises the student to check with their GP.
A worked example
A yoga studio in Norwich gets about 25 new students a month. Each currently fills in a paper health form at the door, which the owner types up.

What it's worth
About 1.25 hours a month saved (75 minutes), worth about £31 at £25 an hour. Zapier's Free plan allows two-step Zaps and 100 tasks a month, so 25 a month fits. Health answers need a Google Workspace business account, not a free one: Google Workspace Business Starter was €6.80 (about £6) per user a month excluding tax on Google's pricing page (workspace.google.com/intl/en_uk/pricing, checked 17 October 2026; the page showed euros, so check the sterling price at sign-up). The owner and two teachers is three users, about £18 a month; one user is about £6.
How to set it up
- Write the form questions with a qualified teacher and keep them to what teachers actually need.
- Build a Google Form in a Google Workspace business account, not a free personal one, with a clear privacy notice and responses going to a Google Sheet shared only with teachers and the owner.
- In Zapier, start a Zap on each new row and send the student a receipt email with no health information in it.
- Test with a dummy response and read the email as a student would.
- Teachers read every response before the first class and talk through any 'yes' answer.
What goes wrong
- Health answers are special-category data under UK GDPR. A free Google account doesn't come with a business data processing agreement, so use Google Workspace (which has Google's data processing terms), accept Zapier's data processing terms, limit who can open the sheet, and tell students how long you keep it.
- A form doesn't replace the teacher's judgement. Software and AI assistants must not tell a student that a class is safe for them.
- Zapier can fail silently. Compare form responses with receipts sent every week.
- Answers go stale; ask again at the start of each term.
