The problem
A client forgets a session and the hour sits empty. If you see 25 clients a week, 2 miss a session and you don't always charge your late-cancel fee, at $150£113€134A$217C$212 a session that's about $1,290£970€1,154A$1,862C$1,823 a month (2 x $150£113€134A$217C$212 x 4.3).
What the AI does
SimplePractice's Essential and Plus plans send appointment reminders by email, text and voice at no extra charge. Clients opt in to the reminders they want, 24, 48 or 72 hours ahead, and can confirm or cancel from a text. Keep the wording discreet: a day, a time and your first name, with no mention of therapy. A client who can't come can tell you in time to offer the hour to someone on your waitlist. TCPA: text only clients who chose texts, and honor STOP.

How to set it up
- Move to Essential or Plus; Starter has no automated reminders.
- Ask each client at intake how they'd like to be reminded (email, text or voice) and record it; never assume a shared phone or family email is private.
- Write reminders with the day, the time and your first name only.
- Turn on text confirmations, and check unconfirmed sessions the day before.
- After two months, compare missed sessions with the two months before.
What goes wrong
- A reminder on a shared phone or family email can reveal that someone is in therapy. Agree the channel with each client and let them opt out.
- If your policy already charges for missed sessions, the gain is fewer awkward conversations rather than more income.
- A client who keeps missing sessions may be disengaging or at risk. Follow it up as a clinician, not as an admin task.
- Promotional texts need prior express written consent under the TCPA. Keep reminders to appointment details.
